According to an accident investigation report, a fatal helicopter crash at Oahu's Wheeler Army Airfield that killed a Chubbuck solider in 2009 may have been caused by pilot error. The Honolulu Star-Advertiser obtained the report, which says the pilots did not correct airspeed to stay within acceptable safety parameters. The May 2009 crash of the helicopter killed Chief Warrant Officer Jonathan Bryce Millward of Chubbuck, and Chief Warrant Officer Stanley Blane Hepfner of Hubbard, Ohio. The flight involved a required test of the helicopter's ability to autorotate, an emergency procedure in the event power is lost during flight. The newspaper reports that while it can't be determined who was at the controls at the time of the crash, the report says Hepfner was the "pilot in command.
